Activating Indigenous Health

All Activate Health’s programs are accessible to Aboriginal and Torres Straight islander peoples, along with specialised services to increase their participation and health. Activate Health strives to ensure all programs are individually centered and goals based, and as such try to include important aspects of culture, family, belonging, and identity that may increase the comfort, participation, and empowerment of individuals and groups.

Chronic Disease Management Program

The Chronic Disease Management (CDM) program is an initiative of Medicare Australia, designed to support individuals with the management of a chronic disease and/or a musculoskeletal injury. The CDM enables a multidisciplinary approach to the treatment of chronic diseases and injuries, to enable long-term resolution of the individual’s condition.

The following medical conditions represent those typically covered under the CDM program (but are not limited to):

  • Weight loss/ weight managements
  • Osteoporosis and osteoarthritis
  • Falls risk and balance training
  • Cancer
  • Cardiovascular disease
  • Diabetes mellitus (Type 2 diabetes)
  • Musculoskeletal conditions such as chronic pain, bursitis, joint degeneration or reduced capacity
  • Stroke
  • Respiratory Diseases

Upon a referral from your GP, you will have access to the services of an ACTIVATE Health Allied health team for up to five (5) visits. In these sessions, our Health Professionals will assess your individual needs, and then tailor an exercise and lifestyle modification program to assist you. Within your exercise sessions, you will experience one-to-one supervision under the care of our Exercise Physiologists, where you will undertake an exercise routine that has been specifically designed to improve your medical condition or injury. In practice treatment and education is performed along with at home independent intervention prescribed such as rehabilatory exercise, daily living modification, lifestyle changes, or health promotion exercise.

Individuals may also move on to regular Activate Health group exercise classes (private paying) following recommendation from the initial consultation, or continue at home independent performance with regular monitoring and support via subsequent one on one consultation.


Aboriginal and Torres Straight Islander follow-up services (81300 to 81360)

ATSI individuals can also access a further 5 allied health follow-up services per calendar year in addition to the Chronic Disease Management plan above.

The 5 services can include:

  • 5 of the same service type, for example, physiotherapy
  • a mix of different service types, for example, 1 dietetic, 2 podiatry and 2 physiotherapy services.

These sessions allow great access to health professionals and allow for increased support for your chronic disease goals.


Group Allied Health Services for Type 2 Diabetes 

The Type 2 Diabetes Group Services initiative provides one Private Consultation and 8 Group sessions for an Exercise Physiologist. These can be used to participate in any of our Activate Health Group Exercise session. The private consultation can be used for health assessment and education, and to determine to class ability type that best suits the patient. These can include in gym classes of different abilities (seated and gentle, up to active and functional) and Hydrotherapy. Classes consist of 2 to 12 participants and are taken by an Accredited Exericse physiologist experienced with exercise and Diabetes management.

My Health for Life

MHFL is a free QLD government Health program for eligible participants performed by local providers such as ACTIVATE Health. 

My health for life is about feeling good and enjoying the benefits of a healthier life. We support everyday Queenslanders to set realistic health goals and empower them with the knowledge, confidence and practical tools to improve their wellbeing. It’s about making healthy choices part of everyday life. There is lots you can do to keep your health on track, as well as reduce your risk of developing serious conditions such as type 2 diabetes, stroke and heart disease. Whether it’s eating well, moving more, maintaining a healthy weight, sleeping better or managing stress, our free, healthy lifestyle program will support you to achieve your goals.

Indigenous and Tores straight islander people are identified as high priority need for this program due to health trends and as such have different eligibility to ensure they are able to assess the program at no cost if needed. This is an important initiative as part of increasing the health and health education of Aboriginal and Torres Straight islander people. This course is heavily lead by the participants and as such can be adapted to the cultural needs of individuals and groups.
